Assessment – What’s involved?
At Notaro Homecare, we want you to feel confident about the care and support you choose, to make it easier we provide advice and a no obligation assessment to make sure our services are right for you.
When you call us we will arrange for our Homecare Manager, Senior Carer and/or Care Coordinator to come and meet with you in person to discuss what type of care and support you require. It’s always a good idea to make sure you have someone with you at this appointment, either a family member or a good friend who has known you a while. During the assessment we will ask you lots of questions to understand the following:
- Personal care and well-being
- Sight hearing and communication
- Medication requirements
- Mental health and cognition
- Risk and personal safety
- Dietary requirements and preferences
- Preferred method of communication
- How you wish to pay for your care and support
Asking these really important questions will help form your comprehensive tailored care plan. The care plan allows us to determine what your wants, needs and what your desired outcomes are and how we achieve them. You are under no obligation to proceed with the package but if you are, we will ask you to formally sign and your care can commence.
Every visit is documented in your care plan by your carers. A full review of your care plan will take place after the first 4 weeks then again after 6 months or should your circumstances change.
During the assessment, you will be given a copy of our Service User Guide, which contains further information about our services and key policies and procedures, including our complaints and compliments procedure.
All information gathered is treated completely confidentially and is never passed to a third party.
